Knee Pain That Isn't From Your Knee: The Hip-Spine Connection

Knee pain with no obvious injury often traces back to the hip and pelvis. Here's how a structural evaluation can find what a knee X-ray misses.

Knee Pain That Isn't From Your Knee: The Hip-Spine Connection

When Your Knee X-Ray Comes Back "Normal"

A lot of patients come to me after an unremarkable knee X-ray, still in pain, and understandably frustrated. If there's no clear injury and the joint itself looks structurally fine, the source of the pain is often further up the chain -- in the hip, pelvis, or even the low back -- creating abnormal forces that show up as knee pain.

How the Kinetic Chain Works Against You

Your knee sits between your hip and your foot, and it has very little ability to compensate for misalignment above or below it. If your pelvis is rotated or one leg is functionally shorter due to structural shift, your knee absorbs the resulting uneven forces with every single step. Over time, that repeated uneven load is a common driver of chronic knee pain that never shows up as damage on imaging.

What This Looks Like Day to Day

  • Knee pain that's worse with walking or stairs but wasn't caused by a specific injury
  • Pain that shifts slightly in location rather than staying pinpointed
  • Pain that seems disproportionate to what imaging shows
  • A history of low back or hip issues alongside the knee complaint

Our Evaluation Approach

We assess your full structure -- pelvis, spine, and lower extremities -- using Advanced Biostructural Correction, because addressing the knee in isolation misses the actual source when the problem originates above it. For active patients and athletes, we often pair this with injury rehabilitation to rebuild proper mechanics once the structural imbalance is addressed.

Not Every Knee Case Is Structural

Direct trauma, ligament injuries, and joint-specific conditions are real and sometimes need their own dedicated treatment or specialist referral. Part of a good evaluation is figuring out honestly whether your knee pain is structural in origin or something that needs a different kind of care.

Runners and Golfers See This Often

Repetitive-motion activities like running and golf are especially prone to this pattern, since they load the hip-knee-ankle chain repeatedly in a way that amplifies even small structural asymmetries.

What a Gait-Focused Evaluation Looks Like

Beyond a standard structural assessment, we'll often watch how you actually walk, since gait reveals compensation patterns that aren't always obvious standing still. Small asymmetries -- one hip dropping slightly more than the other, a foot turning outward, an uneven stride length -- are exactly the kind of details that point us toward the actual source of a knee complaint that imaging alone couldn't explain. We combine those observations with your structural findings to build a plan targeted at the real cause.

Footwear's Role in the Kinetic Chain

Worn-out or unsupportive footwear can amplify an existing pelvic asymmetry, since your shoes are the foundation your entire kinetic chain builds from. We'll ask about your typical footwear during your evaluation, and while replacing shoes alone rarely resolves a structural knee issue, pairing appropriate footwear with structural correction tends to produce more complete, longer-lasting relief than either approach on its own.

Weight-Bearing Activities Worth Reintroducing Gradually

As your structural correction progresses, gradually reintroducing weight-bearing activity -- walking before running, shorter distances before longer ones -- lets your body confirm the correction is holding under real load before you push back to your previous activity level.

A Note for Patients Considering Knee Replacement

If knee replacement has been discussed for wear that's advanced, structural evaluation of the surrounding hip and spine mechanics is still worthwhile, both to support the healthiest possible pre-surgical function and because addressing surrounding compensation can improve overall comfort regardless of what happens with the knee itself.

Find Out Where Your Knee Pain Is Really Coming From

If your knee pain hasn't made sense given your imaging or history, a full structural evaluation may explain what's actually happening. Call (623) 226-7527 or request an appointment.
