SpringBack Chiropractic complies with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its Privacy Rule, which establishes national standards for the protection of certain health information.

Protected Health Information (PHI)

Protected Health Information (PHI) is information relating to an individual's health condition, the provision of health care, or payment for health care that can identify the person. This includes demographic data, names, addresses, birth dates, and Social Security numbers.

Covered Entities

HIPAA applies to health plans, health care providers who transmit health information electronically, and health care clearinghouses. SpringBack Chiropractic operates as a covered entity under this definition.

Your Rights

As a patient, you have the right to:

  • Access your health records
  • Request amendments to inaccurate information in your records
  • Receive an accounting of disclosures of your health information
  • Request restrictions on how we use or disclose your health information
  • Request confidential communications by an alternative means or location

Uses and Disclosures

We may use and disclose your protected health information for treatment, payment, and health care operations without your written authorization. Any other use or disclosure requires your written consent.

Our Administrative Safeguards

SpringBack Chiropractic maintains written privacy policies, designates a privacy official, trains staff on HIPAA requirements, implements physical and technical safeguards to protect your information, and maintains required documentation.

Filing a Complaint

If you believe your privacy rights have been violated, you may file a complaint with us directly or with the U.S. Department of Health and Human Services, Office for Civil Rights. Filing a complaint will not affect your care in any way.
